2. How Does Ovulation Work?
Ovulation sits at the centre of the broader menstrual cycle, which itself comprises two parallel processes: the ovarian cycle and the endometrial cycle. Understanding how ovulation works means following these four phases:

Uterine lining sheds. FSH begins rising, stimulating follicle development.

The Hormonal Cascade
Ovulation is triggered by a surge in luteinising hormone (LH), released by the pituitary gland. This LH surge typically occurs 24β36 hours before the egg is released. In the days leading up to ovulation, rising oestrogen signals the pituitary to flood the bloodstream with LH β the starting gun for follicle rupture. This is the hormone detected by over-the-counter ovulation predictor kits (OPKs).
Once the follicle bursts and releases the egg, it transforms into the corpus luteum (Latin: “yellow body”) β a temporary endocrine gland that produces progesterone. Progesterone prepares the uterine lining for potential implantation and causes basal body temperature (BBT) to rise by approximately 0.2β0.5Β°C, a classic post-ovulation signal.
What Happens to the Egg?
The released egg is captured by finger-like projections called fimbriae at the end of the fallopian tube and transported inward. If sperm are present and one penetrates the egg, fertilisation occurs in the fallopian tube. The fertilised egg (zygote) then travels to the uterus over 3β5 days for implantation. If fertilisation does not occur within 12β24 hours, the egg disintegrates and is reabsorbed by the body. The corpus luteum degenerates, progesterone drops, and menstruation begins approximately 14 days later.
3. When Does Ovulation Happen?
The textbook answer is day 14 of a 28-day cycle, but real-world data tells a far more variable story. A landmark study analysing data from over 32,000 women and 75,000 menstrual cycles found that while the most common ovulation day for a 28-day cycle was day 15, there was still a 10-day spread in observed timing across women.
The 28-day myth: While 25.3% of women believed they had a 28-day cycle, only 12.4% actually did. Most women (87%) have cycle lengths between 23 and 35 days, and over half experience cycles that vary by 5 or more days from month to month.
A major prospective study published in the British Medical Journal β studying 696 menstrual cycles β found that the fertile window occurred across a broad range of cycle days. On every day between days 6 and 21, women had at least a 10% probability of being within their fertile window. In only about 30% of women did the fertile window fall entirely within the days 10β17 window that clinical guidelines often cite.
Factors That Can Shift Ovulation Timing
Even within an otherwise regular cycle, many variables can push ovulation earlier or later: acute illness or fever, psychological or physical stress, significant weight changes, intense exercise, travel, disrupted sleep, and certain medications. Does Ovulation Always Occur?
Not necessarily. An anovulatory cycle is a menstrual cycle in which ovulation does not occur. Bleeding can still happen, but it is not triggered by the usual hormonal sequence. Anovulation becomes more common approaching menopause (average onset around age 51) but can occur at any reproductive age due to hormonal imbalances, PCOS, low body weight, or excessive exercise.
4. The Fertile Window
The fertile window refers to the six days each cycle during which sexual intercourse can result in pregnancy: the five days before ovulation and the day of ovulation itself. This window exists because sperm can survive in the female reproductive tract for up to five days, while the egg is viable for only 12β24 hours after release.
Conception probability is not equal across those six days. Research confirmed that the probability of conception is highest when intercourse occurs the day before ovulation, at approximately 41%. The likelihood drops significantly on the day of ovulation itself and falls to near zero two or more days after ovulation.
Practical takeaway: To maximise chances of conception, aim for intercourse in the 2β3 days leading up to ovulation rather than timing it exactly to the moment of egg release. Having sex every 2β3 days throughout the cycle maintains reasonable conception probabilities without requiring precise real-time tracking.
5. Signs & Symptoms of Ovulation
The body sends several signals around the time of ovulation. Some are subtle; others are quite noticeable. Absence of symptoms does not necessarily mean ovulation is not occurring β many women who ovulate regularly feel nothing at all.

Cervical Mucus Changes

What Causes Ovulation Pain?

First, as the dominant follicle grows (reaching roughly 20mm in diameter) in the days before ovulation, it may stretch the outer membrane of the ovary, causing pressure and discomfort. Second, when the follicle ruptures and releases the egg, a small amount of fluid and blood is released into the abdominal cavity.
