The idea that ovulation always occurs on day 14 comes from the assumption of a perfectly regular 28-day cycle. In this theoretical cycle, the follicular phase (days 1–14) and luteal phase (days 14–28) are equal. In reality, both phases vary considerably between women — and even in the same woman from month to month.

The result: on every day between days 6 and 21, at least 10% of women were within their fertile window. 🌱 Follicular Phase (Days 1 to Ovulation)
This phase begins on day 1 of your period and ends at ovulation. It is the variable half. Length ranges from as few as 10 days to over 21 days. Stress, illness, weight changes, and hormonal fluctuations all affect when the dominant follicle matures and triggers the LH surge. According to research, virtually all variability in cycle length comes from the follicular phase.

A study published in npj Digital Medicine (Bull et al., 2019), analysing over 600,000 menstrual cycles, found that the mean follicular phase length was 16.9 days (range: 10–30 days), while the luteal phase was far more consistent at an average of 12.4 days (range: 7–17 days). This confirms that the follicular phase drives the variability you see in cycle-to-cycle timing.
5. Factors That Shift Your Ovulation Day
Many factors can push ovulation earlier or later within a given cycle — even if you normally have very regular periods. All of these act on the hypothalamus and pituitary gland, the brain structures that control the hormonal signals (FSH and LH) that drive follicle development and the ovulation trigger.
How Stress Delays Ovulation
Stress is one of the most common causes of unexpectedly late ovulation. When you experience significant physical or psychological stress, your body elevates cortisol levels. Cortisol suppresses the release of GnRH (gonadotropin-releasing hormone) from the hypothalamus, which in turn reduces FSH and LH output from the pituitary. Without adequate FSH and LH signalling, follicle development slows and ovulation is postponed.
PCOS and Ovulation Timing
Polycystic ovary syndrome (PCOS) is one of the most common hormonal disorders in women of reproductive age and is a leading cause of irregular or absent ovulation (anovulation). In PCOS, elevated LH and testosterone levels disrupt the normal follicle selection process, often resulting in multiple small follicles that fail to reach maturity and ovulate. Women with PCOS may ovulate infrequently, unpredictably, or not at all in some cycles.
6. Does Ovulation Happen Every Month?
Not necessarily. A cycle without ovulation is called an anovulatory cycle. You can still experience menstrual-like bleeding during an anovulatory cycle — the bleeding is driven by estrogen fluctuations rather than the normal progesterone-driven withdrawal — but it is not true menstruation, and pregnancy is not possible.

After ovulation, progesterone from the corpus luteum causes a slight but sustained rise in basal body temperature (BBT) of 0.2–0.5°C. If you are charting your temperature and see no sustained rise across your cycle, this may indicate anovulation.
🧪 No Positive OPK Reading
Ovulation predictor kits (OPKs) detect the LH surge that triggers ovulation. If you test daily and never see a positive result during your cycle, this could indicate that the LH surge — and therefore ovulation — did not occur.
🩺 Low Mid-Luteal Progesterone
The most definitive clinical confirmation of ovulation is a mid-luteal serum progesterone test (taken around day 21 of a 28-day cycle). A level above 3 ng/mL confirms ovulation occurred. A low level suggests anovulation.
7. How to Know When You Are Ovulating
Because ovulation timing is so individual and variable, the most reliable approach is to combine two or more tracking methods rather than relying on calendar counting alone.
Method 1 — Ovulation Predictor Kits (OPKs)
OPKs detect the LH surge in urine, which occurs 24–36 hours before ovulation. Research from the big-data study (Rowland et al., 2020) found that a detectable LH surge occurred in 97.9% of cycles when women tested consistently each day. This makes OPKs the most reliable at-home method for predicting when ovulation is about to happen — not just estimating it from cycle length.
Method 2 — Cervical Mucus Monitoring
As ovulation approaches, cervical mucus becomes clear, stretchy, and slippery — resembling raw egg white. This change typically begins 2–3 days before ovulation and signals that the fertile window is opening. After ovulation, progesterone causes mucus to become thick and opaque again. Monitoring this change is free and can give 1–3 days of advance notice before an LH surge.
Method 3 — Basal Body Temperature (BBT) Charting
After ovulation, the corpus luteum releases progesterone, which raises resting body temperature by approximately 0.2–0.5°C. Tracking this rise over multiple cycles helps confirm that ovulation is occurring and reveals your typical ovulation timing pattern. Note that BBT confirms ovulation after it has occurred — it cannot predict it in real time.
